Humana Releases New
Medicare ID Cards for 2006
Starting on January 1, 2006,
Humana members began receiving new, redesigned Medicare ID cards.
The goal for the redesign was to make the cards simple, clear and
senior-friendly. There are several elements of the card that are
of special interest to the provider and remain on the front of the
card, including copayment information, effective date and member
ID number. On the back of the card, providers will find supplemental
benefit information, the claims submission address and the authorization/notification
phone number.
The three key elements identified as most important
to the member appear prominently and in bold font. This includes
member name, member ID number and customer service phone number.
All the cards meet Centers for Medicare & Medicaid Services
(CMS) requirements.
All of the ID cards have the same design; the only variation is
driven by product type— HMO, PPO, PFFS, PDP and Group. Any
plan that has a pharmacy component has either affinity or pharmacy
network logos on the front of the card. Affinity logos represent
companies with which Humana has an affiliation. To aid in reducing
variations of PPO ID cards, all contractually required regional
provider network information appears on the back of these cards.
Below are samples of two of the new member ID cards, with some of
the key elements highlighted.
For more information on the new ID cards,
call the customer service number listed on the back of the card.
